Job Description:
Key Responsibilities:
* Conduct thorough assessments of children and young people with additional needs.
* Develop and implement personalized intervention plans.
* Provide training and upskilling to families, caregivers, and other professionals.
* Deliver effective and evidence-based interventions.
* Manage and efficiently close episodes of care.
* Contribute to the ongoing development and improvement of our services.
* Visit community sites, including schools and homes, to provide on-site support.
Essential Criteria:
* Experience working with children and young people with moderate to complex additional needs at a Band 7 level.
* Proven ability to assess, plan, implement, and evaluate interventions.
* Excellent time management skills and the ability to manage a caseload effectively.
* Strong communication and teamwork skills.
* A valid driving license and access to a car for community visits.
