Job Description:
Our Client, Liverpool City Council, is seeking an Adults Social Worker to join its AMHP Team.

Responsibilities of this role:
* Working with adults who are diagnosed with mental health conditions in secondary mental health care, including: dementia, psychosis, anxiety and depression.
* Work as part of a multi-disciplinary Community Mental Health Team alongside health and AHP colleagues.
* Knowledge and experience of implementing the Care Act and Mental Capacity Act is essential.
* Work directly with service users and their carers using a range of knowledge, skills and interventions as required, developing an individualised approach to the practical and functional needs of the service users.
* Recognise various mental health problems and undertake or participate in holistic needs based assessments, produce a care plan, implement and review the plan, including the management of risk.

* Work at the service works within the Care Program Approach and sound risk management skills are necessary.
Requirements of this role:
* Must be a Qualified Social Worker, registered with Social Work England.
* Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
* Must have minimum of 2 years experience working as a Qualified social worker in a similar team
