As the Care Coordinator, you will have front-line responsibility for managing our dedicated support team, ensuring they provide exceptional care to individuals in their homes across the Buckinghamshire area. Reporting to the Registered Manager, you will play a crucial role in planning, supervising, and delivering personalized care that meets the highest standards.
Key Responsibilities:
* Collaborate as a senior team member to provide care and support for service users.
* Plan and implement safe working practices for the home care team.
* Provide leadership, instruction, and supervision for a team of care workers.
* Supervise care delivery and prepare detailed care plans tailored to individual needs.
* Conduct initial visits to service users to create comprehensive care plans and regularly monitor and review these plans.
* Arrange appropriate care packages in line with individual needs, ensuring the quality and suitability of care provided.
* Monitor service delivery according to Care Standards, ensuring regular feedback to the Registered Manager.
* Carry out detailed risk assessments to minimize risks to service users, workers, and the public, with regular reviews and information sharing.
* Utilize care management software to ensure timely coverage of service user’s bookings.
* Deliver ongoing formal and practical training to team members, maintaining accurate training records.
* Supervise and monitor team members, taking appropriate actions as required.
* Develop care plans that outline specific care and support needs.
* Lead team meetings, inductions, supervisions, and appraisals.
* Maintain personal skills in delivering home care/community support by monitoring care workers as required.
* Maintain accurate records related to attendance, care plans, and risk assessments.
* Participate in Out of Hours System/On Call support, providing assistance outside regular office hours.
* Engage in team, multi-agency meetings, and relevant external meetings.
* Maintain open communication channels within the Home Care Service and with other agencies, ensuring effective partnerships.
* Liaise with local community organizations and stakeholders to build strong relationships.
* Attend training courses to maintain professional development and competency.
* Ensure staff deliver high-quality, personalized care to clients.
* Adhere to our client’s policies, procedures, National Care Standards, and Health and Safety regulations.
* Maintain confidentiality of information acquired during duties.
* Undertake other tasks as reasonably requested.
* Commit to a minimum of 15 hours of care each week to support your team, while promoting recruitment in your area.
* Ensure that activities such as personal care and medication administration meet required standards.
* Schedule staff rotas, carry out spot checks, annual appraisals, interviews, risk assessments, and care plans for new clients. Ensure your team is fully trained and complete all paperwork for auditing.
Qualifications and Experience:
* Proven experience in a similar role within the care sector.
* Strong leadership and supervisory skills.
* Excellent communication and interpersonal abilities.
* In-depth knowledge of care planning, risk assessments, and regulatory standards.
* Proficiency in care management software.
* Ability to work independently and as part of a team.
