Service Manager, £450pd, Hospital Discharge, Liverpool
Pertemps are recruiting for a highly experienced Service Manager for the Integrated Hospital Pathways Service. The Service Manager will be responsible for leading a city-wide hospital discharge service working closely with system partners to reduce length of stay in acute settings and promote the home first ethos.
The post holder will support in the development of the Transfer of Care Hub and Pathways 1, 2 and 3. They will work collaboratively with service users and carers groups and staff at all levels within the service and partner organisations to develop effective reablement and strength-based services that focus on improving the outcomes of people and achieve best value.
The role is agile with the requirement to be in the office 2 days per week.
