Job Title: Mental Health Practitioner

Ward: PCMHT South Southwark
Shift Patterns: Monday - Friday 9.00am - 17.00pm

Job Responsibilities:
* Front door assessment and liaison mental health service
* Quick, timely interventions, crisis planning and risk assessment, management of patients in the community for up to 12 weeks
* Duty rota-take part in duty rota and screen/decline referrals that come via the inbox
* Crisis plan and risk assessment-complete 7 day follow ups, liaise closely with the ward, HTT and AMHP service
* Work closely with psychology services and refer to appropriate therapy pathways
* Take part in busy MDT, including attending meetings, reflective practice session and governance meetings
What you’ll need:
* The creativity to solve problem and tackle obstacles in new ways
* Experience working as a care coordinator
* Post registration experience in mental health care
* Registered with correct body
