Social Worker - Hospital Discharge Assessment Team
Up to £32 per hour
Location:Kings Mill Hospital, Sutton-in-Ashfield
About the Role:
We are seeking a skilled and experienced Social Worker to join our Hospital Discharge Assessment Team. You will play a crucial role in facilitating timely and safe discharges from Kings Mill Hospital.
Key Responsibilities:
*Conduct comprehensive assessments of patients’ social and care needs upon hospital admission.
*Develop and implement discharge plans, coordinating with multidisciplinary teams to ensure seamless transitions of care.
*Liaise with community services, care providers, and other relevant agencies to secure appropriate post-discharge support.
*Advocate for patients’ needs and rights, ensuring they receive the best possible care.
*Participate in a rota system to cover the Transfer of Care Hub at Kings Mill Hospital.
*Maintain accurate and up-to-date records.
*Promote effective communication and collaboration with healthcare professionals, patients, and families.
Essential Requirements:
*Social Work Degree (or equivalent)
*HCPC Registration
*Significant experience in a hospital or community setting
*Strong assessment and care planning skills
*Excellent communication and interpersonal skills
*Ability to work independently and as part of a team
*Flexibility to work shifts, including weekends and evenings
Desirable Requirements:
*Experience in discharge planning and complex case management
*Knowledge of local health and social care services
*Ability to manage a demanding workload
