Position: Hospital Discharge Social Worker
Pay Rate: £35-£38 per hour
Contract: Temporary, Full-time
Responsibilities:
*Coordinate safe and timely discharges from hospital
*Assess the needs of patients and their families
*Develop and implement care plans
*Liaise with health professionals and community services
*Provide support and advice to patients and their families
Requirements:
*Degree in Social Work
*Registered with the Scottish Social Services Council (SSSC)
*Experience in hospital discharge planning
*Knowledge of relevant legislation and policies
*Excellent communication and interpersonal skills
