About the Role:
We are seeking an experienced Advanced Clinical Practitioner to join an Urgent Community Response Team in Kirklees. This role is pivotal in providing high-quality care to patients in their own homes and care homes, ensuring timely and effective clinical interventions.
Key Responsibilities:
*Conduct initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
*Demonstrate safe, clinical decision-making and expert care, including advanced assessment and diagnostic skills.
*Provide care for patients in their own homes and care homes.
*Support in an acute environment when required to facilitate discharge.
Requirements:
*Proven experience as an Advanced Clinical Practitioner.
*Ability to work within professional boundaries to provide comprehensive patient care.
*Strong clinical decision-making skills.
*Flexibility to work weekends and evenings as required.
