Job Summary
A fantastic opportunity has come up for a full time Pharmacist to work for Ramsay Health Care UK based primarily at the Berkshire Independent Hospital in Reading.
The Berkshire Independent Hospital is part of a small cluster group of Ramsay Hospitals which includes Ashtead Hospital and North Downs Hospital in Caterham. There may be some cross site working to align pharmacy services, support individual training courses plus support to the other pharmacy teams.
The post holder will be a member of the pharmacy team and will provide a clinical pharmacy service to the inpatient wards and outpatient department. The aim is to promote the safe, effective, economical use of medicines in patients being cared for by the Hospitals, in accordance with the required standards of quality and professional practice, to meet the needs of the users.

Main duties
*To provide an exemplary pharmacy service in accordance with current legislation, accepted professional and ethical standards and clinical requirements.
*To provide advice and support to consultants, other clinicians and patients ensuring the safe, appropriate and cost effective use of medicines.
*To advise the hospital and its departments on the safe management of medicines, ensuring compliance with current legislation and professional standards.
*To promote a multidisciplinary, patient focused approach to the delivery of an efficient, high quality healthcare service.

Job Description
*Participate in the clinical ward pharmacy service covering the inpatient wards and outpatients
*To deliver a clinical pharmacy service to the defined clinical areas which will include safely and efficiently undertaking: drug history taking, medication reconciliation, utilisation of patients own drugs, self-medication schemes, discharge counselling, medicines optimisation. Additionally supporting / advising the ward on medication safe and secure storage and handling.
*Make appropriate referrals where necessary; ensure the clear and effective communication of patients’ pharmaceutical needs to other pharmacists and health care professionals both within the trust and in other health care environments.
*Provide prescription review and medicines-related advice to healthcare professionals at ward level in a range of acute specialties, with advice from Senior Pharmacists where required.
*Provide timely and accurate advice on prescribing issues and evidence-based prescribing
*Identify and resolve (with advice from Senior Pharmacists where required) prescribing-related problems to ensure the principles of clinical governance and the safe and effective use of medicines are followed.
*Provide medicines-related information to members of the multidisciplinary ward team, and specific advice on drug administration to nursing staff.
*Work at ward level and in the dispensary to ensure prescriptions are dispensed safely and effectively, and provide professional intervention including clinical screening, dispensing and final checking of prescriptions, including those used off label and unlicensed medicines
*Assist in the provision of Medicines Information services; Provide timely and accurate advice and information to enquiries made by nursing, medical and other health care staff in order to ensure continuity of the service and the safe, effective and economic use of medicines.
*Work effectively as part of a team in a multidisciplinary and pharmacy environment, and maintain rapport with medical and other colleagues.
*Achieve objectives agreed at the Personal Development Review in order to contribute effectively to the provision of pharmacy services that are continuously improved, and to ensure continuous personal and professional development. Effectively, efficiently and actively liaise with all relevant staff to ensure objectives in all key result areas are achieved.
*To improve clinical skills and expertise in orthopaedic, bariatric and ophthalmic post-surgery recovery including pain management, DVT risk management etc.
*Identify areas suitable for clinical audit and Drug Utilisation Reviews (DUR) and undertake audit to help rectify the issues.
*Identify therapeutic areas of cost and service improvement.
*Assist in the training of junior pharmacy staff e.g. pre-registration pharmacists/pharmacy students/student technicians and other healthcare staff e.g. RMO’s and nursing staff.
*Ensure regular updating by self-directed learning or the attendance at such courses or establishments that is of benefit to the service.
*Maintain a CPD portfolio and take part in an organised development programme which will include regular updating/training sessions in other area of the department.
Essential:
Degree in Pharmacy
Member of GPhC Ggeneral Pharmaceutical Council) with current registration
Willingness to undertake clinical training
Demonstration of up to date general clinical awareness
Understands importance of monitoring quality of service
Knowledge and understanding of medicines legislation
Awareness of current developments in Pharmacy practice
Dispensing experience.
Patient counselling experience.
Experience of supervising and training staff.
Multi-disciplinary & team working
Evidence of good clinical knowledge and skills
Good communication skills – both written and verbal
Ability to work under pressure.
Ability to prioritise work effectively
Good organisation, time and management skills.
Team player with good problem solving skills and ability to work independently and as part of a team
Drivers License with access to a car
Desirable:
Graduate with some hospital experience
Evidence of commitment to CPD
Member of RPS
Royal Pharmaceutical society)
Post Graduate Diploma in Clinical Pharmacy or equivalent
Experience in ward/clinical pharmacy and medicines information
Familiarity with JAC (CMM) computer system
Working in hospital pharmacy
Previous audit work
Basic understanding of IT systems
Awareness of current issues in hospital pharmacy
