Location: Droitwich
Position Type: Full-time

Contract: Ongoing (Locum, 3 month minimum)
Are you an experienced Occupational Therapist looking to make a difference in the community? We have an exciting opportunity for a Band 6 Occupational Therapist to join our dynamic team in Droitwich Spa.
Key Responsibilities *Lead and manage discharge planning for patients transitioning from hospital to home or community care.
*Provide tailored occupational therapy interventions to support patient recovery and independence.
*Work collaboratively with multidisciplinary teams to deliver person-centered care.
*Contribute to the development and implementation of care plans that optimize patient outcomes.
Essential Requirements *Degree in Occupational Therapy with current HCPC registration.
*Proven experience in discharge planning in a healthcare setting.
Desirable Skills *Previous experience in a community-based role.
