Job Overview We are seeking an experienced Band 6 Occupational Therapist to join the team at Manchester University NHS Foundation Trust. The role involves conducting specialist assessments and discharge planning within an acute medicine environment. Key Responsibilities *Conduct detailed assessments to identify patients’ needs in an acute medical setting.
*Collaborate with patients, families, and multidisciplinary teams to plan and implement effective discharge strategies.
*Offer expert advice on rehabilitation and ongoing care plans to ensure continuity and quality of care.
*Maintain accurate and thorough documentation in line with Trust policies.
*Actively participate in case reviews, team meetings, and service improvement initiatives.
Requirements *HCPC registration as an Occupational Therapist.
*Demonstrated experience in acute medicine, particularly in assessment and discharge planning.
*Strong clinical reasoning and decision-making abilities.
*Excellent communication and teamwork skills.
*Dedication to delivering compassionate, patient-centered care.
