Location: Halesfield, Telford, Shropshire Community Health NHS Trust
Pay: £20 to £23 per hour
Pay Mechanism: Direct Engagement
Contract Duration: 03/12/2024 to 03/03/2025

Job Overview We are seeking a motivated and experienced Band 6 Occupational Therapist to join the community team at Shropshire Community Health NHS Trust. The role involves delivering patient-centered care in a community setting, supporting rehabilitation and promoting independence.
Responsibilities *Manage a caseload of patients in the community, assessing their functional needs and developing tailored intervention plans.
*Conduct holistic assessments of patients’ physical, cognitive, and emotional needs.
*Work collaboratively with multidisciplinary teams to deliver integrated care.
*Provide rehabilitation to patients with complex conditions, enabling them to achieve independence in daily activities.
*Deliver equipment recommendations and adaptations to improve patients’ quality of life.
*Offer education and advice to patients, families, and carers to support long-term well-being.
*Maintain accurate documentation in line with Trust policies and procedures.
Requirements *HCPC registration as an Occupational Therapist.
*Experience in a community setting is preferred.
*Strong communication, organizational, and problem-solving skills.
