What You’ll Be Doing: Join our team as a Band 7 Clinical Pharmacist and play a key role in improving the lives of patients across primary and secondary care. In this role, you will:
*Perform comprehensive medicines reconciliation on admission
*Deliver medicines optimisation & clinical services during inpatient stays
*Attend ward rounds/MDTs to support patient care
*Provide pharmacy authorisation of discharge summaries
*Work across multiple sites (travel required)
*Develop innovative practices to enhance patient experience and care
Requirements & Qualifications: To be successful in this role, you will need:
*A current and valid GPhC registration
*Experience in general medical specialties
*Strong communication and teamwork skills
*Ability to work independently and within a multidisciplinary team
*A commitment to high-quality patient care
Working Hours & Contract Duration: This role offers a flexible working position requiring availability from Monday to Friday. The contract is ongoing, with a strong possibility of extension based on performance and service needs.
