Job Description:

We are seeking a dedicated Young Carer Practitioner to join our team. This is a fantastic opportunity to support young carers and their families, helping them build confidence, independence, and resilience.
What You’ll Be Doing:*Deliver direct support to children, young people, and families through one-to-one and group sessions.
*Complete whole-family assessments and develop tailored support plans in collaboration with key agencies.
*Facilitate weekly social groups and the Young Carers Council.
*Raise awareness of young carers’ needs across services through training and outreach.
*Work to reduce inappropriate caring responsibilities by ensuring adult needs are met.
What We’re Looking For:*Strong communication skills and experience working with children, families, and professionals.
*Ability to assess, plan, and review support within safeguarding frameworks.
*Experience with targeted interventions and group facilitation.
*Good IT skills, including Microsoft Office and database management.
*Understanding of GDPR and data protection principles.
*Willingness to develop professionally and work flexibly, including hybrid arrangements.
*Driver preferable
