Job Description:
Our Client, Oldham council is seeking an Adult’s Social Worker to join their CMHT.

Responsibilities of this role:
*Conduct assessments of adults with mental health needs under the Care Act 2014.
*Develop, implement, and review care and support plans in collaboration with service users and other professionals.
*Undertake safeguarding enquiries and risk assessments.
*Work closely with psychiatrists, nurses, occupational therapists, and other CMHT members.
*Support hospital discharge planning and prevent unnecessary admissions.
*Apply the Mental Capacity Act and, where appropriate, the Mental Health Act.
*Maintain accurate and timely case records and reports.
*Advocate for service users’ rights and promote independence and recovery.
*Liaise with families, carers, and external agencies to coordinate care.
Requirements of this role:
*Must be a Qualified Social Worker, registered with Social Work England.
*Must have minimum of 3 years post qualifying social work experience
