Our Client, Barnsley Council, is seeking an Adults Advanced Practitioner to join their Team.

Responsibilities of this role:
*Conduct comprehensive assessments of adult patients’ physical, emotional, and social needs to develop a deep understanding of their individual circumstances.
*Coordinate and facilitate safe and timely discharge plans.
*Provide advocacy and support to patients and their families.
*Conduct risk assessments and implement safeguarding measures to ensure the well-being and safety of vulnerable adults during the discharge process.
*Respond to urgent requests to visit anywhere in the borough.
Requirements of this role:
*Must be a Qualified Social Worker, registered with Social Work England.
*Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
*Must have minimum of 3 years’ experience working as a Qualified social worker in a similar team
