Locum Social Worker - Leaving Care Team
Are you a passionate and committed Qualified Social Worker with experience supporting care-experienced young people?
We are currently recruiting for an experienced Leaving Care Social Worker to join a supportive and forward-thinking Leaving Care Team in Halton. This is a rewarding opportunity to work with young people as they transition into independence, helping them achieve positive and sustainable outcomes.

Responsibilities: *Manage a caseload of care-experienced young people
*Develop and review Pathway Plans in line with statutory requirements
*Support young people in areas such as accommodation, education, training, and employment
*Promote independence and life skills development
*Work collaboratively with partner agencies and professionals
*Maintain accurate and timely case recordings and reports
*Advocate for young people, ensuring their voice remains central
To be considered for the role, ideally you will have: *Diploma / Degree in Social Work
*Social Work England registration
*Experience working with Leaving Care / Care-Experienced Young People
*Strong understanding of relevant legislation and statutory duties
*Ability to build effective relationships with young people
*Good organisational and communication skills
*Ability to work independently and as part of a team
