Job Description:
Job Title: Social Work Assistant
Department: Adult Social Services
Location: Cardiff (office-based with travel across Cardiff)
Reports to: Team Manager
Hours: 37 hours per week
Job Purpose This Social Work Assistant role is based in Cardiff within an busy Adult Services Review Team. You will work closely with the Team Manager and Social Workers to support the delivery of effective adult social care services across Cardiff, helping to coordinate care and ensure support plans are implemented and reviewed.
Key Responsibilities * Arrange and monitor services in line with individual care plans
* Provide practical advice and support to service users and carers
* Liaise with voluntary and statutory agencies
* Complete care plan reviews and reassessments
* Participate in team meetings and duty rotas
* Follow organisational policies and safeguarding procedures
* Engage in ongoing training and professional development
Person Specification Essential
* Experience working with older adults and/or adults with physical disabilities
* Strong communication and organisational skills
* Ability to work independently and as part of a team
* Commitment to equality, inclusion, and empowering service users
Desirable
* NVQ/GNVQ Level 3 or 4 in Social Care
* Experience working with multi-agency teams or healthcare settings
Additional Information The role is primarily office-based with occasional visits to service users. A full driving licence and access to a vehicle during working hours is desirable.
This role requires a strong commitment to safeguarding and promoting the wellbeing of vulnerable adults. Appropriate background checks may be required.
