Contract: Agency Interim
Rate: £250 per day (Umbrella)
Service: Families First - SEND Education, Health & Care Assessment & Planning
We are recruiting an experienced SEND Key Worker to join Staffordshire’s SEND EHC Assessment & Planning Team on an interim basis.
This role focuses on processing Annual Reviews, Phase Transfers, EHCP amendments and drafting new Education, Health & Care Plans (EHCPs) in line with SEND legislation and statutory timescales.
Key Responsibilities*Coordinate and facilitate EHC needs assessments and outcome-focused planning
*Process Annual Reviews and Phase Transfers within statutory deadlines
*Amend and draft high-quality, person-centred EHCPs
*Analyse multi-agency advice (Education, Health, Social Care, Educational Psychology)
*Act as a key worker for children and young people with SEND and their families
*Broker provision and support packages to meet agreed outcomes
*Maintain accurate electronic case records and performance data
*Contribute to safeguarding and multi-agency working in line with statutory duties
Essential Requirements*Significant experience within SEND / EHCP statutory services
*Strong working knowledge of the SEND Code of Practice, Children Act and Education Act
*Experience planning personalised support for children or young people with disabilities
*Ability to manage a caseload, prioritise workload and meet deadlines
*Strong report writing and IT skills
*Confident working collaboratively across education, health and care partners
