What you will do:*Work as a Street Healthcare Outreach Nurse alongside homelessness outreach professionals to address complex health needs.
*Carry out dynamic risk assessments to ensure personal and partner safety in line with organisational policies.
*Build therapeutic relationships using trauma-informed approaches to enable effective assessment and treatment.
*Deliver advanced homeless healthcare, making autonomous clinical decisions and complex interventions when required.
*Act as a health case manager, setting and achieving health outcomes for individuals with high unmet needs.
*Conduct holistic health assessments, identifying physical, mental health, addiction, safeguarding, housing, and social care needs.
*Carry and utilise clinical equipment during street shifts to provide immediate care.
*Identify and respond to clinical risks, including escalation to emergency services or prescribing where appropriate.
*Complete mental capacity assessments and manage care refusals in line with organisational policy.
*Provide primary care treatments on the street and support integration into mainstream healthcare services.
*Deliver wound care, dressings, and treatments under Patient Group Directions, maintaining communication with healthcare partners.
*Advocate for access to primary care, reduce reliance on emergency services, and address safeguarding concerns.
*Support housing pathways by working closely with outreach workers and partner agencies.
*Establish and maintain partnerships with health, housing, and social care services to ensure coordinated care.
*Facilitate case conferences and information sharing in line with governance and data protection principles.
*Deliver health screenings, vaccinations, and support integrated care pathways for communicable diseases.
*Safely perform phlebotomy and ensure appropriate handling and follow-up of results.
