Our client, the Royal Borough of Windsor and Maidenhead, is seeking an agency Social Worker for its Hospital Discharge team.


This is a 3-month post initially, paid at up to £35 per hour.


The role will involve the following:
*Provides a range of support and care services for older people, adults with a physical and learning disability and people with mental health needs both in the community and at home.
*Hospital discharge work

Requirements of the role:
*Qualified Social Worker
*Fully Social Work England registered
*Must have significant post-qualified experience in adult social care
*Muat have general hospital discharge experience
