In this role, you’ll be working over seven days a week to make a genuine difference in healthcare by designing interventions and assessing risks for residents at various stages of the hospital pathway.

What you will do:
- Support residents in risk assessment and intervention design within the hospital pathway.
- Contribute to the Discharge to Assess pathway through referral documentation screening and planning.
- Perform post-discharge assessments in residents’ homes, providing therapy interventions to support recovery and independence.
- Consider environmental factors, provision of equipment, minor adaptations, and manual handling to optimise resident care.
- Collaborate with residents on discharge pathways to create Reablement Plans and Goals, focusing on strength-based practice.
- Embrace flexible working arrangements with a mix of home and office-based tasks.
Requirements:
- Must be registered with the Health and Care Professions Council (HCPC).
- Car driver preferred, to facilitate client visits and enhance service delivery.
