Key Responsibilities:
*Delivering care plans for patients and ability to work efficiently within a busy general ward/department
*Monitoring vital signs
*Observing patient’s condition and recording information
*Communicating with patients and their families
*Supporting with personal care, comfort and dignity
*Maintaining accurate care records and reporting concerns to senior staff
*Working collaboratively with the day & night team to ensure a safe, calm environment
*working in a nursing home supporting residents with dementia, palliative care, stroke & complex care needs


Essential Qualifications & Experience:
*Care Certificate, Health & Social Care Certificate or equivalent
*At least 6 month’s experience working in NHS hospital, nursing home, care home & hospice supporting residents with dementia, palliative care, stroke & complex care needs within the UK.
*A caring, reliable and patient approach
*Excellent communication skills, both verbal and written, with the ability to build rapport with patients.
*Ability to work effectively within a team while also demonstrating initiative in patient care.
*Compassionate approach with strong interpersonal skills, demonstrating empathy towards patients’ experiences.
*Proficiency in IT systems for maintaining patient records and accessing medical information.
*right to work in UK
*Enhanced DBS clearance required.
