Experienced Occupational Therapist - Reablement

An experienced Occupational Therapist is required to join a busy Reablement Team within a local authority Adult Social Care service. The team focuses on supporting individuals to regain independence and reduce the need for long-term care.
Key Responsibilities
*Manage a complex caseload of adult clients
*Complete occupational therapy assessments and develop reablement plans
*Carry out moving & handling and risk assessments
*Recommend equipment, adaptations and Disabled Facilities Grants
*Work collaboratively with health professionals, families and partner agencies
*Provide professional support and guidance to colleagues
*Maintain accurate records and reports
Requirements
*Degree or Diploma in Occupational Therapy
*HCPC registration
*Minimum 2 years post-qualification experience
*Experience working with adult clients and complex needs
*Knowledge of health, housing and social care legislation
*Experience with adaptations, reablement and moving & handling
Additional Information
*Enhanced DBS required
*Travel required for home visits
