Home Support Matters - Triage Care Worker
Are you an experienced and confident care professional who can make quick, effective decisions? We’re seeking skilled and compassionate care workers to join our team, delivering high-quality care during peak demand periods.
What You’ll Do
Play a critical role in supporting new customers integration, ensuring excellent standards of care from day one of a customer’s journey with HSM.
Your responsibilities will include:
*Providing short-term care and support, focusing on new customer integration.
*Building strong rapport with customers and their families from the outset.
*Identifying risks, safeguarding concerns and any required interventions.
*Communicating clearly with branch teams, field care supervisors and other professionals.
*Ensuring accurate, timely documentation that meets regulatory standards.
*Representing Home Support Matters with professionalism, empathy and expertise.
What You’ll Bring
*Strong experience in care or support roles.
*Confidence in assessing needs and making informed decisions.
*Excellent communication and calm under pressure.
*Solid understanding of safeguarding and risk.
*A proactive, organised and professional approach.
*A Full UK Driving Licence and access to own vehicle with business insurance.
