Job Description:
Location: Blackpool, Adult Social Care
Job Type: Contract
Hybrid Working: Office presence required at least 3 days a week
We are seeking a motivated and compassionate Case Assessor to join our community social work team within the Adult Social Care division. This role offers the opportunity to make a significant impact in the lives of adults in the Blackpool community by using a strengths-based approach to promote independence and prevent future care needs.
Day-to-day of the role:
*Undertake needs-led assessments and reviews, constructing, monitoring, and reviewing appropriate outcome-based packages of care in accordance with departmental policies and procedures.
*Work closely with social work colleagues, other teams, providers, carers, and external agencies to provide comprehensive and seamless services across adult social care.
*Empower individuals by supporting them to make informed choices, including undertaking capacity assessments and arranging for advocates to work with individuals as appropriate.
*Maintain up-to-date knowledge of current developments relating to social care policy and interventions.
*Advocate on behalf of service users and ensure the service is delivered in compliance with the Care Act (2014).
Required Skills & Qualifications:
*GSCEs/NVQ2/Level 2 Diploma or equivalent qualification including Maths and English.
*Substantial knowledge of the key elements of the Care Act (2014) and its impact/implications on practice.
*Experience in a social care setting and inter-agency working.
*Strong communication skills – both verbal and written, with good IT skills.
*Ability to form good working relationships with service users and accurately gather and present information.
