The people we support may have spent an extended period in hospital or another setting, therefore we utilise a person-centred approach; empowering individuals to become as independent as possible within their own homes
About the Role
Join a dedicated team committed to making a real difference in the lives of individuals with complex mental health needs. You’ll work directly within the community, supporting people in their own homes to regain independence and confidence, guided by personalised plans and backed by expert clinical support. This role offers meaningful, hands-on experience and ongoing training, empowering you to develop valuable skills in trauma-informed care and recovery-focused support. Job Opportunity
Provide personalised, community-based support to individuals with complex mental health needs, promoting independence within their own homes. Work closely with clients to implement and review tailored care plans, ensuring their safety and well-being. Collaborate with multidisciplinary teams, including clinical professionals, to deliver trauma-informed and recovery-focused care. Maintain accurate records and report any changes in client condition or concerns promptly. Participate in ongoing training to develop skills in mental health support and person-centred approaches. Essential qualifications include strong communication and interpersonal skills, empathy, reliability, and a willingness to learn and work flexibly within a community setting. Previous experience in social care or mental health support is desirable but not essential.

*Previous experience in social care or mental health support is desirable but not essential
*Competence in maintaining accurate records and reporting changes in client condition promptly
*Commitment to ongoing professional development and training to enhance skills and knowledge
