We are looking to recruit a Senior Practitioner – Referral and Assessment within the Social Care industry in Hammersmith and Fulham. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels. The Senior Practitioner – Referral and Assessment vacancy in Hammersmith and Fulham will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.

Responsibilities include:
*Undertaking initial investigations
*Undertaking core investigations
*Operating under the remit of the Children’s Act, 1989
*Managing a high caseload
*Maintaining accurate and up-to-date case files
*Contributing to the duty rota
*Supporting junior members of the team

Experience preferred:
*Post qualifying experience of working as a Senior Practitioner – Referral and Assessment within the UK Social Care industry.

Minimum Requirements:
*HCPC Registration
*4 years post qualifying experience within the UK Social Care industry
*Enhanced DBS/CRB (we can obtain for you)
*Eligibility to work in the UK
