Full job description Key Responsibilities
*Carry out holistic assessments of children and young people with disabilities in their home environments.
*Asses, recommend and provide specialist equipment to support daily living and independence i.e. specialist seating, moving and handling, bathing and toileting.
*Assess for and contribute to home adaptations, ensuring environments are safe, functional, and fit for purpose.
*Work closely with families and carers, providing guidance, reassurance, and practical support.
*Collaborate with multi-disciplinary professionals across

*Health (e.g., NHS colleagues)
*Education providers
*Partner and voluntary agencies
*Contribute to care planning, ensuring interventions are child-centred and outcome-focused.
