Job Description:
Full job descriptionOur client Cambridgeshire county council is looking for an Adults Social Worker to join their Learning Disabilities team.
Key Responsibilities*Complete Care Act 2014 assessments, including complex and high-cost care packages
*Undertake timely reviews following changes in individuals’ needs or circumstances
*Deliver a minimum of two completed and recorded assessments per week
*Carry out face-to-face assessments and reviews
*Actively involve advocates, carers, and families
*Analyse and evaluate care and support arrangements
*Work closely with brokerage and contracts colleagues
*Ensure care packages maximise independence and wellbeing
*Maintain accurate and defensible records
Essential Requirements*Qualified Social Worker with Social Work England registration
*Experience with Learning Disabilities/Autism
*Knowledge of Care Act 2014 and Mental Capacity Act
*Experience reviewing complex care packages
*Access to own vehicle is essential
