Job Description:
Full job descriptionAs a Commissioning Broker, you’ll be at the heart of connecting people with the care they need. Acting as the link between families, professionals, and providers, you’ll help secure high-quality care packages and placements that truly make a difference.
In this role, you will:
• Support safe and timely hospital discharges, working closely with partners to ensure everything is in place
• Manage a diverse and sometimes complex caseload
• Maintain accurate and up-to-date records across systems
• Handle sensitive information with professionalism and confidentiality
• Monitor provider performance to maintain high standards of care
• Work collaboratively with managers to improve processes and outcomes
This is a fast-paced, meaningful role where your work has a direct impact on people’s lives every day.

• Strong communication and relationship-building skills

• Excellent attention to detail, particularly in administrative tasks
