Occupational TherapistLocation: Thackley Green Specialist Care Centre, North Northamptonshire
Hours: 37 hours per week
About the RoleWe are seeking a motivated and compassionate Occupational Therapist to join our team at Thackley Green Specialist Care Centre. This is an exciting opportunity to play a key role in supporting patient recovery and enabling safe, timely discharges from hospital settings.
You will lead therapeutic assessments and rehabilitation for patients transitioning from hospital into our service, working collaboratively within a multidisciplinary team (MDT) to deliver person-centred care and achieve positive outcomes.
Key Responsibilities*Lead comprehensive therapy assessments and rehabilitation programmes for patients admitted from hospital
*Conduct home access visits to assess the need for minor adaptations and equipment
*Effectively manage your own caseload, prioritising and planning interventions to meet patient needs
*Delegate tasks appropriately to Therapy Assistants, ensuring safe and efficient service delivery
*Support the wider Discharge to Assess pathway, taking on referrals where required
*Work closely with multidisciplinary professionals to facilitate smooth and timely discharges
*Promote independence and support individuals to return safely to their home environment
*Full UK driving licence with access to a vehicle and appropriate business insurance (essential
