Contract role, Discharge to Assess
Pertemps are recruiting for experienced Adult’s Social Workers in Bristol
This is to work in a Hospital Team across 3 Hospitals.
The Role:
*Assessment and Review
*Post hospital discharge
*CAA and Support planning
*BI
*MCA, Safe Guarding
*Duty cover
*Attend QA panel for funding provision.
