Hours: 37.5 hours per week | Rota Basis (Monday-Sunday)
About the RoleWe are currently recruiting for an experienced Support Worker / Project Worker to join a supported accommodation service supporting individuals with complex needs. This is an excellent opportunity for someone with experience in homelessness, supported housing, mental health, or complex needs services who is passionate about helping people achieve independent living.
Working as part of a dedicated team, you will manage a small caseload, deliver person-centred support, complete assessments, and work collaboratively with partner agencies to achieve positive outcomes for residents.
Shift PatternThe role operates on a rota basis across Monday to Sunday, including:
Key Responsibilities*Manage a caseload of approximately 7 residents, delivering regular one-to-one key work sessions.
*Develop and review person-centred support plans and safety plans with residents.
*Complete comprehensive needs and risk assessments using appropriate assessment tools.
*Deliver trauma-informed and psychologically informed support that promotes dignity, safety and independence.
*Support residents with budgeting, tenancy sustainment, daily living skills and establishing positive routines.
*Encourage engagement with education, employment, training, volunteering and community activities.
*Build effective relationships with external agencies including health services, mental health teams, social care and community organisations.
*Support residents to attend appointments and access specialist services.
*Maintain accurate case records, support plans and reports in line with organisational policies.
*Contribute to the safe and effective day-to-day operation of the service while following safeguarding and health & safety procedures.
Requirements*Experience managing caseloads within supported housing, homelessness, mental health or complex needs services.
*Experience completing needs, support and risk assessments.
*Good understanding of safeguarding, confidentiality, health & safety and supported housing procedures.
*Knowledge of trauma-informed and person-centred approaches.
*Strong communication and relationship-building skills.
*Ability to work collaboratively with internal teams and external professionals.
*Excellent record-keeping, IT and report-writing skills.
*Ability to work flexibly across a rota, including evenings and weekends.
Desirable*Experience supporting people experiencing homelessness or multiple disadvantage.
*Knowledge of housing support, tenancy sustainment or supported accommodation services.
*Understanding of mental health, substance misuse and safeguarding practices.
