We are looking to welcome a caring, dynamic and highly motivated Support Coordinator to join our well-established Supported Living Service in Doncaster. This is an exciting opportunity to be part of a team that believes in people, celebrates achievements, and supports individuals to live fulfilling, independent lives.
As a Support Coordinator, you will play a key role in coordinating support for 30 individuals across 15 supported living and outreach services in Doncaster. Working alongside colleagues, families and external professionals, you will ensure people receive consistent, high-quality support that enables them to achieve their goals and live meaningful lives.
You will champion a strengths-based approach, helping people to develop life skills, build relationships, pursue hobbies and interests, access education/employment opportunities, and become active members of their local communities.
About You
The successful candidate will be passionate about delivering outstanding person-centre care and support, and demonstrate unconditional positive regard for the people they support. You will bring excellent communication and relationship building skills and a positive, solution-focused approach. You will need to be organised, resilient, and able to manage multiple priorities across a number of services.
Experience supporting people with learning disabilities and/or complex needs and completing needs assessments, support planning and risk assessments are essential.
Applicants must be able to demonstrate flexibility, as the role involves working shifts over a seven-day rota and participating in the local on-call service.
Key Responsibilities
*Support and coordinate services for 30 individuals with learning disabilities and mental health needs.
*Develop, review and monitor person-centred support plans and risk assessments.
*Build positive relationships with the people we support, their families, advocates, and external professionals.
*Ensure that support plans promote independence, strengths, aspirations, and positive outcomes.
*Monitor service quality to ensure consistently high standards.
*Support staff teams to deliver compassionate, outcome-focused support.
*Identify opportunities for community participation, education, employment and social inclusion.
*Respond proactively to changing needs while promoting positive risk-taking and effective risk management.
