Responsibilities of this role:
*Undertake Care Act assessments, reviews, and person-centred support planning for autistic adults and individuals with a range of care and support needs.
*Promote independence, wellbeing, and positive outcomes through strengths-based, neurodiversity-affirming social work practice.
*Manage safeguarding concerns, risk assessments, and interventions in line with relevant legislation and local procedures.
*Apply the Mental Capacity Act 2005 and support individuals with decision-making and best interest processes where required.
*Work collaboratively with service users, families, carers, health professionals, and partner agencies to coordinate effective support and access to community resources.
*Maintain accurate case records, reports, and statutory documentation while managing a diverse and complex caseload.
Requirements of this role:
*Must be a Qualified Social Worker, registered with Social Work England.
*Must have minimum of 3 years post qualifying social work experience
