We are currently recruiting for an experienced Adult Social Worker to join a Hospital Team in Merton.
The role will involve working with adults in a hospital setting, supporting assessment, discharge planning and positive outcomes for individuals returning to the community or moving into appropriate care.
Key responsibilities:
*Undertake Care Act assessments and reviews
*Support hospital discharge planning
*Assess care and support needs and develop person-centred plans
*Work collaboratively with health and social care professionals
*Safeguard adults and respond to identified risks
*Liaise with families, carers and partner agencies
*Maintain accurate records and work within statutory timescales
Requirements:
*Social Work England registration
*Qualified and experienced Adult Social Worker
*Experience within hospital or discharge services is desirable
*Strong knowledge of the Care Act and adult safeguarding
*Ability to manage a busy caseload effectively
