Our client

Morgan Hunt is working with a homeless prevention charity based in Greater Manchester, dedicated to providing shelter and support to vulnerable individuals in the community. The Complex Needs Navigator will play a crucial role in managing a caseload of individuals facing challenges such as rough sleeping and addiction, ensuring they receive the tailored support necessary for their well being and stability.



The role;
*Manage a caseload of up to 25 individuals, serving as their primary key worker to provide tailored support across Oldham.
*Establish and maintain trusting, trauma-informed relationships with individuals facing complex challenges.
*Facilitate access to essential services, including housing, healthcare, and social care.
*Assist with homelessness assessments and develop Personal Housing Plans.
*Create practical move-on plans, addressing barriers to securing safe housing.
*Conduct strengths-based needs and risk assessments and coordinate multi-agency support plans.
*Advocate for clients, addressing service barriers and enhancing local service pathways.
*Work flexibly across various services and locations to ensure accessible support.

The candidate
*Proven experience working with individuals facing complex needs such as homelessness, addiction and mental health.
*Ability to develop and implement person-centered support plans.
*Experience in conducting needs and risk assessments.
*Familiarity with safeguarding practices and procedures.
*Strong background in client advocacy and navigating service systems with the confidence to professionally challenge when required.
*Experience with multidisciplinary teams and partner agencies.
*Knowledge of local housing and social care services.

Education requirements
*NVQ Level 3 in Health & Social Care (or similar) or demonstrable experience across the Social Care sector.



As this is a community based role, candidates will required access to a car for work alongside business insurance and an Enhanced Adult DBS.
