Our North & Mid Outreach team operate throughout the Burgess Hill/Haywards Heath/Crawley/East Grinstead areas so being a car driver is essential for this role.

About the RoleAs part of our outreach team, you will:
*Provide personalised, community-based support to individuals with complex mental health needs, promoting independence within their own homes.
*Work closely with clients to implement and review tailored care plans, ensuring their safety and well-being.
*Collaborate with multidisciplinary teams, including clinical professionals, to deliver trauma-informed and recovery-focused care.
*Maintain accurate records and report any changes in client condition or concerns promptly.
*Participate in ongoing training to develop skills in mental health support and person-centred approaches.
*Essential qualifications include strong communication and interpersonal skills, empathy, reliability, and a willingness to learn and work flexibly within a community setting.
*Previous experience in social care or mental health support is desirable but not essential.

*Basic understanding of mental health, trauma-informed care, and recovery principles is advantageous.
*Competence in maintaining accurate records and reporting changes in client condition promptly.
*Commitment to ongoing professional development and training to enhance skills and knowledge.


Essential:
*Car owner/driver is essential for this role.


Desirable:
*Experience supporting individuals with mental health conditions in community settings.
